Etymology: In reference to the state of Utah, and Greek, erpeton, "crawling."

Etymology:

Holotype: BYUG 3050

Locality: Clay pit in sec. 9, T5S, R1W, 3 km north of highway 73 and 16 km west by northwest of Lehi, Utah County, Utah.

Horizon: Uppermost units of the Manning Canyon Shale Formation.

Biostratigraphy:

Age: Upper Chesterian, Serpukhovian Epoch, Late Mississippian or lower Morrowan, Bashkirian Epoch, Early Pennsylvanian, middle Namurian, lower Silesian series.

Material: Fragmentary skull and fragmentary skeleton.
